a. Every job-related injury should be reported as soon as possible to the supervisor or Human Resources rep. This also includes and illness or disease that may be caused by employment.
b. The Date of Injury is the 1st day you realize your symptoms or disability is a direct result of that injury.
c. Ask your supervisor or Human Resources rep to authorize medical treatment by signing and providing form CA-16. This form authorizes treatment for up to 90 days while the claim is under development. By the end of the 90 days, the employee will either need to return to work, or he/she will need a CA-7 to get more treatment. The CA-7 also should be used to claim continuing compensation, when a previous CA-7 claim has been made.
d. Obtain one of two forms that will be presented for your injury: CA-1 for traumatic injuries -or- CA-2 for occupational disease.
a. If you have been injured as a federal employee, you are entitled to select a treating physician. It is important to find out which doctors in your area are experienced with DOL claims. At Return To Health, our staff starts treatment immediately, on good faith, prior to claim approval. They can also assist the patient in filling out the CA2 report to be sure everything is properly filled out to expediate your claim.
b. CA-20 form – Attending Physician Statement – This report provides the OWCP with the details of the injury, any examination findings, diagnosis, prognosis, and a treatment plan.
c. Dr. Garrett understands the federal worker’s compensation program and will help you navigate through this process so you can get the benefits you deserve.
d. You can change your treating doctor at any time if you inform OWCP in writing. Be sure to provide the doctor's name and address and reason for the change.
e. Obtain the OWCP-957 form allows the patient mileage reimbursement for travel to and from the medical provider, to and from any therapy treatments, and to and from the pharmacy for medicine used for the injuries.
Be sure to obtain a receipt of Notice of Injury for your records. .

At this time, your Supervisor and Dr. Garrett will provide OWCP with CA-17 Duty Status Report outlying any restrictions or inability to work information.
Dr. Garrett conducts monthly follow ups with his patients to make sure their claim doesn’t fall through the cracks.

If your claim is denied, you can request reconsideration in writing and send the OWCP a new medical report.
