➢ Every job-related injury, including any illness or disease that may be caused by your employment, should be reported as soon as possible to your supervisor or Human Resources representative.
➢ Obtain Form CA-1 (for traumatic injuries) or Form CA-2 (for occupational diseases, conditions, or repetitive use injuries) for submission to the OWCP (Office of Workers’ Compensation Programs).
➢ Your “Date of Injury” for a CA-1 injury is the date of the traumatic incident and for a CA-2 injury is the first day you realize your symptoms or disability is a direct result of your on-the-job injury.
➢ For traumatic injuries, ask your supervisor or Human Resources representative to authorize medical treatment by signing and providing Form CA-16. This form authorizes treatment for up to 60 days while your claim is under review.
➢ Select a treating physician who understands federal workers compensation claims, and who can assist you with the claims process. Dr. Spicer, Dr. Garrett, and the staff at Return to Health will start treatment immediately (even prior to your claim being approved by the OWCP), and they will assist you in filling out your Form CA-1 or CA2 correctly to expedite your claim.
➢ Select a treating physician who understands federal workers compensation claims, and who can assist you with the claims process. Dr. Spicer, Dr. Garrett, and the staff at Return to Health will start treatment immediately (even prior to your claim being approved by the OWCP), and they will assist you in filling out your Form CA-1 or CA2 correctly to expedite your claim.
➢ Your treating physician will complete a Form CA-20 (Attending Physician Statement) to provide the OWCP with the details of your injury, medical examination findings, diagnoses, prognoses, and your treatment plan.
➢ Complete Form OWCP-957 which allows you to get mileage reimbursement for travel to and from your medical providers, therapy treatments, and pharmacy.
➢ Keep copies of all documents submitted.
➢ Immediately provide your claim number to your health care providers.
➢ If your claim is denied, you can request reconsideration in writing and send the OWCP a new medical report. Dr. Garrett is skilled in the reconsideration process, and you will be able to continue treatment at Return to Health while your claim is under review.
➢ You will be able to receive treatment as long as your treating physician can demonstrate to the OWCP that the treatment is helping you get better.
➢ Once your treating physician has determined that your condition is not going to continue to improve, Return to Health will provide an impairment rating for determination by the OWCP of a scheduled monetary award to you.
